I confirm that I own the pet or I was given authority by the owner for taking ownership of the pet.

I assure Bonehemian Tails that this pet is currently in good health condition.

I confirm that my pet has a complete and updated vaccination.

I acknowledge that the pet should have a safety collar.

I confirm that my pet doesn't have any flea or any communicable disease.

I release Bonehemian Tails , its employees, and owners from any liabilities but not limited to injury, sickness, damage, accident, or death while in the vicinity.

If ever that I wasn't able to pick up my pet on time, Bonehemian Tails will be calling the owner and get authorization for the pet to stay with Bonehemian Tails.

If my pet needs medical attention, I authorize Bonehemian Tails to have it look by the veterinarian nearest to the location.

Pet Parent or Owner agrees to be fully responsible for the cost of any such medical treatment and for the cost of any transportation for the purposes of such treatment.

Owner or Pet parent  understands special-needs dogs, young puppies, and senior dogs naturally have a higher risk of injury, stress-related illnesses, weakened immune system, or exacerbation of any pre-existing condition. As such, by using our facility for day care or boarding the Owner or Pet Parent is waiving any claim for injury or illness experienced by Owner’s dog while in our care

I certify that I have read and understand the rules and regulations herein and that I have read and understand this this agreement. I agree to abide by the rules and regulations and accept all terms, conditions, and statements of this agreement and confirm the truthfulness of the contents of the application form completed by me. I am of the full age of eighteen years of age.
I confirmed that all information I entered in this form is true and accurate.

 

Every Saturdays and Sundays Bonehemian Tails under goes deep cleaning which is herbal process and isnt perniciousness/harm your furry buddy we do this to disinfect the home for ticks ,lices and other airborne diseases or diseases harmful to our pets who will be boarding long or short terms including day care.

 

If your dog does have lice feel free to be honest and open with the Owner of Bonehemian Tails she will undertake to clean and remove lice's or ticks residing in the in skirts of your dog. Honesty and openness is much appreciated at Bonehemian Tails as Owners of this canine business we are in the interest and welfare of our beautiful dogs and don't judge the owners on basis of anything.

If your fur buddy is infested with ticks and fleas and is boarding at Bonehemian Tails we will do the necessary treatment for the ticks and fleas ,the cost will be billed to the pet parent/owner. In the interest of the other dogs and the pet /buddy being board ,Bonehemian tails has every right to medicate and disinfect the dogs.
These insects feed on your pet’s blood and can cause health problems ranging from allergic reactions to serious tick-borne illnesses. In order to prevent

If the owner or pet parent decides to abandoned their pet at Bonehemian Tails action will be taken against the Pet Parent and the Dog will be placed in a good home. Please Note Animals are living beings also so please respect their right to life and existence consequences will be uphold on the actions you leave behind.

If a fur buddy is been abused by the Pet parent or owner legal authorized action will be taken by Bonehemian Tails. No kind of intolerance will be accommodated by the Pet Parents as the safety of all animals are Bonehemian Tails priority.
